Sportsguard and the Nottinghamshire Football Association work together to ensure players can play the beautiful game without the ongoing worry of injuring yourself during play - if you happen to obtain an injury of an applicable nature during a game of play, here is some information which may help in making a claim.

All Injury claims MUST be reported within 30 days from the injury and all claim forms and relevant documentation MUST be returned within 60 days of receiving the claim form.
 
All claims must be reported and claim forms requested by the club secretary or the club contact who is registered with Sportsguard. Claims will NOT be accepted direct from a player as it is a club Policy.
 
If a player has a bad injury and expects to be off of work for a long period he can request interim payments by ticking the relevant box on the claim form.
 
In order for a claim to be handled quickly and efficiently all of the questions on the form must be completed and all requested documentation must accompany the claim form.
